Step 1 – Complete the Submission Form
Download and complete the MedX Analytical Sciences Inc Sample Submission Form. This form provides the information our laboratory needs to correctly identify, process, and report your samples.
The Company / Organization Name entered on the submission form will be used on your laboratory documentation and final report.
Step 2 – Authorize Payment
Payment authorization must be completed before laboratory processing begins.
Secure Online Payment
Submit your payment information through our secure payment system. Payment information is processed securely through our payment provider.
MedX Analytical Sciences Inc does not store complete credit card information.
Charges will be processed according to the testing services requested and your submitted samples.
Alternative Payment Authorization
If another payment arrangement has been approved, include the required authorization information with your submission.

Step 3 – Prepare Your Samples
- Ensure all sample containers are securely sealed.
- Clearly label each sample with the correct identification information.
- Confirm that sample IDs match the submission form.
- Use appropriate packaging to protect samples during transportation.
- Follow any sample-specific handling or storage requirements provided by MedX Analytical Sciences Inc.
Step 4 – Ship Your Samples
Please include the following with your shipment:
- Completed Sample Submission Form
- Required payment or payment authorization
- Properly labeled samples
- Any additional documentation requested for your testing service
